(For enrolment under the Chief Minister’s Comprehensive health Insurance Scheme in tamil Nadu)

To apply for and enrol in the health scheme — which provides cashless medical treatment through empanelled hospitals — you need to carry the following documents at the time of application and verification:

📑 Primary Documents (Mandatory)

Income Certificate

Issued by the Village Administrative Officer (VAO) or Revenue Authorities.

This proves eligibility based on annual family income.

Ration Card / Family Card

The family card must include your name and other family members.

Carry both the original and a photocopy.

Self‑Declaration by Head of Family

A signed statement confirming family details and eligibility.

Aadhaar Card

Aadhaar is used as identity and address proof for all beneficiaries.

📄 Additional Identity & Address Proofs (Carry Along)

These are typically needed during enrolment verification at the centre:

Identity Proof (any one, if Aadhaar isn’t enough)

  • Voter ID, Passport, Driving Licence, PAN Card, etc.

Address Proof

  • Often the same Aadhaar or ration card; PAN may support verification.

PAN Card (optional but useful)

  • Especially when income verification is needed.

📌 Where and How to Submit These Documents

Visit the nearest authorised enrolment centre with all documents.

Submit originals and copies for verification.

Your biometric details (photograph, fingerprints, eye scan) will be taken.

Once verified, you will receive your CMCHIS smart card (also called the Amma Maruthuva Kapitu Thittam Card), which you can use for cashless treatment at empanelled hospitals.

 Tips Before You Go

  • Make sure name spelling on Aadhaar and ration card matches.
  • Keep originals and copies ready to speed up enrolment.
  • If you don’t have a PAN card, it’s usually okay as long as identity and income proofs are clear.
